1. SERVICES

Company agrees to provide pest control services as selected below:

[ ] Essential Plan -- $69/month. Quarterly exterior perimeter treatment.

[ ] Preferred Plan -- $89/month. Quarterly interior + exterior, seasonal mosquito, termite monitoring.

[ ] Premier Plan -- $119/month. Bi-monthly full perimeter, priority scheduling, annual WDO.

[ ] One-Time Treatment -- $[____] for single service of [type].

[ ] Commercial Program -- $[____]/month for [frequency] service.

2. TERM AND BILLING

Initial Term: 12 months, beginning [Start Date].

Billing: Monthly via auto-pay on [Day] of each month.

Annual Pre-Pay Option: Customer may pay annually and receive 10% discount.

Late Fee: $15 if payment is not received within 5 days of due date.

3. RE-SERVICE GUARANTEE

Company guarantees that if covered pests return between scheduled visits, Company will re-service at no additional charge within [5 business / same week] days. This guarantee applies only to plan subscribers and covers pests listed in the service schedule. Exclusions: [bed bugs, termites, wildlife unless specifically covered].

4. CUSTOMER RESPONSIBILITIES

Customer agrees to:

• Provide access to property at scheduled times

• Maintain reasonable sanitation and structural conditions

• Notify Company of pest activity within 48 hours of observation

• Keep pets and children away from treated areas until dry (typically 2 hours)

• Not apply competing pesticides without notifying Company

5. CANCELLATION

Customer may cancel after the initial 12-month term with 30 days written notice. If Customer cancels during the initial term, an early termination fee of [50% of remaining contract value / $150 / no fee] applies. Company may terminate this agreement with 30 days notice for non-payment or unsafe conditions.

6. LIABILITY AND INSURANCE

Company carries general liability insurance of $2,000,000 per occurrence. Company is not liable for: pre-existing damage, structural defects, or customer negligence. Customer's sole remedy for service failure is re-service or refund of last payment.

7. TERMITE BOND (If Applicable)

[ ] Essential Bond -- $799/year. $50,000 repair coverage. 1 annual inspection.

[ ] Preferred Bond -- $1,199/year. $100,000 repair coverage. 2 inspections. Transferable.

[ ] Premier Bond -- $1,499/year. $250,000 repair coverage. 2 inspections + moisture. Transferable.

Bond covers subterranean termite damage to structural members. Damage must be confirmed by Company inspector. Repair coverage is limited to the selected tier maximum.

8. AUTO-RENEWAL

This agreement automatically renews for successive 12-month terms unless either party provides 30 days written notice of non-renewal. Pricing for renewal terms may increase by no more than [5% / 10% / CPI + 3%] annually.
