Template Overview
This template combines the visual aid (the laminated three-tier sheet) with the verbal script that the Care Plan Specialist (CPS) uses to present the recommended care plan after the provider has completed the Report of Findings (ROF).
The structure is built on three clinical tiers (Resolution / Restoration / Wellness), each priced and positioned to give the patient a deliberate choice between three meaningful options — never one option, never five. Three tiers consistently produce 70-85% close rates and average case values 2-3x higher than single-option pitches.
This template is paired with SOP-12 (Financial Presentation) — read that SOP for the full conversation flow. This document specifies the visual layout and the exact tier-presentation script.
The Three Clinical Tiers
Tier 1: RESOLUTION — "Get Out of Pain"
Clinical positioning: Targeted intervention to resolve the chief complaint. Pain reduction, basic functional restoration.
Typical duration: 8-12 visits over 6-8 weeks

Tier 2: RESTORATION — "Resolve and Rebuild" (RECOMMENDED)
Clinical positioning: Full structural correction. Resolves the complaint AND retrains the body to hold the corrected pattern.
Typical duration: 18-24 visits over 12-16 weeks
Typical investment: $2,997 - $3,997
Includes:
18-24 in-office sessions
Two formal re-evaluations (week 4, week 12)
Custom rehabilitation exercise program with progression

Tier 2 (Restoration) is visually emphasized — 10% larger column, "★ MOST POPULAR" tag, slightly different border color (warm gold or brand accent).
Tier 1 left, Tier 3 right — but verbal presentation always starts with Tier 3 (anchor effect).
Prices large and bold — easy to scan.
Payment-option boxes are checkboxes the CPS will tick during the close.
Guarantee block visible but understated — risk-reversal lives at the bottom.
Patient signature line turns the document into a commitment artifact.
Print 100 copies on high-quality paper, laminate, and store at every CPS station.
The Verbal Presentation Script
Use exactly with SOP-12. Here's the tier-walk:
Setup — Before The Tiers
"[Patient first name], Dr. [Provider] just walked me through what they found and what they're recommending for you. Before we look at numbers, let me make sure we're aligned on three things. First — your goal was [restate verbatim future-state from intake]. Second — Dr. [Provider] explained what's actually causing this is [paraphrase root cause]. Third — the recommended approach is [restate the plan]. We on the same page on those three?"
Wait for "yes" on all three. Do not proceed without alignment.
"Perfect. Here's how the recommended program looks across three different ways to approach it. Same provider, same evidence-based work — three different depths of care."
CPS slides the laminated sheet across the desk. Patient and decision-partner see it together.
Tier 3 First — Wellness (Anchor)
CPS points to right column.
"Some of our patients choose Wellness. Same active care as the recommended program, plus a full year of monthly maintenance afterward. The investment is $5,247 — about $437 a month for 12 months. Patients who choose Wellness rarely have a recurrence — they tell us it's the best money they've ever spent on their health. They never have to do this work again."
Brief pause.
Tier 2 — Restoration (Recommended)
CPS points to middle column.
"Most of our patients choose Restoration. Same depth of care, no long-term maintenance — 18 to 24 visits over about 16 weeks. We solve the structural pattern, you graduate, and you're done. Investment is $3,497, or three monthly payments of $1,165."
Brief pause.
Tier 1 — Resolution (Floor)
CPS points to left column.
"And some patients prefer to start small. Resolution is targeted just at the pain — 8-12 visits over 6-8 weeks, $1,997. It gets you out of pain, but I want to be honest with you — it doesn't address the structural pattern that caused this in the first place. About 30-40% of patients who choose this end up coming back within 12 months for the deeper work."
Pause. Silence. Do not speak first. Average silence here: 12-30 seconds.
Closing Question
"Looking at these three, [Patient first name] — knowing what your goal is and what Dr. [Provider] just told you about your situation — which one feels like the best fit?"
Wait. Whoever speaks first loses. Most patients select Tier 2 (Restoration) within 30-90 seconds.
If patient hesitates or asks questions, route to:
"Can I get insurance to help?" — Branch to insurance script (SOP-12 objection 4)
"It's more than I expected" — Branch to value-stack script (SOP-12 objection 2)
"Can I do half?" — Branch to integrity script (SOP-12 objection 3)
"Let me think about it" — Branch to 1-10 scale question (SOP-12 objection 5)
"I need to ask my spouse" — Branch to dial-in script (SOP-12 objection 1)
Tier-Mix Calibration
Track tier-selection mix monthly. Healthy mix:
